Supported Living Offer
Within our CQC registration we also offer a supported living service, enabling people to live in their own home, or a shared house within the local community. Support is assessed and provided for personal care, and day to day living activities, and a bespoke package is agreed to reflect the number of support hours required. Accommodation is arranged through local housing associations or private landlords and agreed within a separate contract. Occupants are supported by our team to maintain their tenancies.
A separate assessment process is required for a supported living offer to be made, and of course dependent upon accommodation being available and funding being in place. Please contact us if you are interested in this aspect of our provision.
Transition Offer
We are developing a Transition Service, and intend to offer this service from Autumn 2024.
Our Transition programme offers a tailored care and support package aimed to support and promote independence and develop skills for daily living.
